What is Living My Life?
Living My Life is an ACC disability support service delivered in the community for kiritaki (clients) with long-term disabilities following an injury. The service is focused on enabling self-determination, so each person has choice and control over where they live, what they do, where they go, and who they connect with.
Service Philosophy
The philosophy at the heart of Living My Life is to support kiritaki to lead lives that are meaningful to them. This means:
Taking a person-directed approach, ensuring individuals make decisions about the supports and providers that help them participate in activities that matter most.
Adopting a whole-of-life focus, so supports adapt as people's roles and experiences change over their life journey.
Embracing diversity, recognising whānau, cultural identity, and self-determination for each person as a valued member of their community.
What Does the Service Provide?
Living My Life offers four flexible, personalised components that are matched to each person’s aspirations and goals:
Facilitated Pathway Map: Helping kiritaki and whānau co-design a visual map of their hopes and goals for the future.
Independent Facilitation: Assisting kiritaki to explore their support options and make informed decisions about who provides their support.
Tailored Supports: Providing ongoing, practical support to build life skills, strengthen community participation, and increase independence.
Coaching to Self-Manage: Supporting kiritaki to learn about managing their own services, developing skills for self-direction, and taking greater control of their support and funding.
Who is Living My Life For?
This service is for kiritaki whose injury has impacted their ability to participate in everyday life, including challenges in community engagement, work, relationships, independence, or managing major life changes. The service works in genuine partnership with individuals, their whānau, ACC, and the community, respecting cultural values, aspirations, and all contributions.
